Appointments

To schedule an appointment, call our main office phone number. We have two office locations:

Office hours

Our office is open Monday to Friday from 9 to 5.  We see patients Tuesdays from 2 to 5 at the Coral Way location and Thursdays and Fridays from 2 to 5 at the Mercy location.

Insurance

We are providers of most insurance plans, including medicare.  If your insurance requires a referral, make sure that you bring it with you or fax it to our office prior to your appointment. Co-payments and deductibles are the responsibility of the subscriber and are due at the time services are provided.

Self pay

We offer financing for self pay patients. Call our office for prices and credit applications.
